Termination of Eligibility for Non-Medicare Eligible Retirees and Eligible Dependents

 

If you or your eligible Dependent fails to pay the required self-payments, coverage for you and your Dependents will terminate at the beginning of the month for which the required contribution was not paid.

When you or your Dependents become eligible to enroll in Medicare for any reason, coverage for you and your eligible Dependents will end under this Plan (regardless of whether you and your Dependents actually enroll in Medicare) unless you and your Medicare eligible Dependents enroll in the SMW+ Plan described in Eligibility Requirements for Medicare Eligible Retirees.

Your surviving eligible spouse or eligible Dependents will remain covered by this Plan as long as they continue to meet the eligibility requirements.

If you become eligible for Medicare and you fail to enroll in the SMW+ Plan, your coverage and coverage for your eligible Dependents will end under this Plan and will not be reinstated.
