If you are a Non-Bargained Employee, you and your Dependents are subject to a preexisting condition limitation, which limits coverage for your preexisting conditions to a maximum of $2,000 per month for six months following your enrollment date.

You may reduce or eliminate the length of the six-month limitation period by providing the Fund with a Certificate of Creditable Coverage showing that you or your Dependent had prior creditable coverage and that there was no break in such coverage of 63 days or more.

A “preexisting condition” is one for which medical advice, diagnosis, care, or treatment was recommended or received within a six-month look-back period. The look-back period ends on the day before your enrollment date.

For purposes of this provision, “enrollment date” means the earlier of the date that your coverage first begins, or the date that the waiting period for such coverage first begins. Also, for purposes of this provision, “creditable coverage” means coverage under another group health plan for which you did not experience a break in coverage of 63 days or more.
