The Optical (Vision) Benefits are payable for:

1. Eye examinations by an ophthalmologist (M.D.) or licensed optometrist.
2. Lenses prescribed by an ophthalmologist (M.D.) or a licensed optometrist, including prescription sunglasses, prescription transitional lenses,
3. Frames purchased in conjunction with lenses newly prescribed by an ophthalmologist (M.D.) or a licensed optometrist.
