Termination of Eligibility – Non-Bargained Member

Your coverage will terminate at the end of the month in which:

• You stop working in an eligible classification, or in the geographical jurisdiction of Sheet Metal Workers’ Local 73, which means in Cook County or Lake County, Illinois.

For example, if you stop working in the geographic jurisdiction of Local 73 in April, then your eligibility for benefits will end on May 31st.

• Your Employer fails to make timely contributions, by the end of the month of coverage, on your behalf.

For example, if your Employer does not make the required contribution on your behalf by April 30th, then your eligibility will terminate on April 30 th.

If you retire, coverage will terminate, but you and your Eligible Dependents may continue coverage under COBRA, as provided on page 21. You may choose to terminate coverage under the Plan when you reach Medicare eligibility.
