Once you meet the Comprehensive Major Medical Benefits deductible, the Plan pays benefits for mental health treatment as follows:

Inpatient (limited to 30 days per calendar year)

PPO Facility . . . . . . . . . . .80% of covered expenses
Non-PPO Facility . . . . . . .70% of covered expenses

Outpatient (limited to 45 visits
per calendar year) . . . . . . . . . . . .50% of covered expenses
