If you are a new or returning employee, your coverage becomes effective after you have worked at least 200 hours in a two-consecutive month period for which your employer is required to contribute to the Welfare Fund. Your coverage will begin on the first day of the second month following this two-consecutive month period. For example, if you work a total of 200 hours during August and September, your coverage will begin on November 1.

Once you are eligible, you'll continue to be eligible if you work 100 hours per month in covered employment or 1,700 hours in covered employment in the preceding twelve-consecutive month period. For example, if you work 100 hours in August or you have at least 1,700 hours in the twelve months ended August 31, you will remain eligible for the month of October. If you work 100 hours in September or have at least 1,700 hours in the twelve months ended September 30, you will remain eligible for the month of November.

Your coverage will terminate at the end of the month following the month you fail to have at least 100 hours or active service, unless you had at least 1,700 hours of active service in the prior 12 months. For example, if you fail to work 100 hours in October, but had 1,700 hours from November through October 31, you would be eligible in December. If you did not have 100 hours in October or 1,700 hours in the 12 months ended October 31, your coverage would terminate at midnight on November 30.

IF ELIGIBILITY ENDS

If your eligibility for benefits ends (regardless of how long you have been eligible), you may become eligible by meeting the initial eligibility rules again. If your eligibility for Plan C benefits ends, you will be notified of any COBRA continuation coverage or any other extended coverage options that are available to you and your eligible dependents.

ELIGIBILITY FOR YOUR DEPENDENTS

As an active employee under the Welfare Plan, you, your legal spouse and your unmarried eligible dependent children are eligible for these benefits as long as you are eligible. Eligible dependent children are your natural or legally adopted children or stepchildren to age 19 (age 23 if he/she is a full-time student) who are domiciled with you and who depend on you for support.

However, before claims for your dependents will be paid, you must complete an enrollment form and provide verification of dependent status to the Fund Office. Verification includes your marriage certificate for your spouse and birth certificates for your children.
