Infertility Treatment Benefits cover in vitro fertilization (IVF), embryo transfer, artificial insemination (AI), uterine embryo lavage, gamete intrafallopian tube transfer (GIFT), zygote intrafallopian tube transfer (ZIFT) and lower tubal transfer once you meet the Comprehensive Major Medical Benefit deductible.
