You must call BlueCross BlueShield of Illinois for pre-certification of any hospital admission, at 1.800.255.5192:

• At least one business day before a scheduled hospital admission; or
• Within two business days of an emergency or maternity hospital admission (the hospital, physician, or a family member may call for you).

If you do not call BlueCross BlueShield of Illinois when required, your benefits will be reduced by $250.

The Plan covers a wide variety of services and supplies that are medically necessary for the treatment of non-occupational sickness and accidents. You are encouraged to use PPO facilities whenever possible so you can receive full Plan benefits and receive the PPO "preferred" rates.
