Hearing Aid Maximum . . . . . .$1,300 or $650 per ear (during a consecutive three-year period)

Payment will be made for a hearing aid only if:

• an examination indicates the need for a hearing aid; and
• the examination and the hearing aid are both provided by a doctor or audiologist who is certified by the American Speech-Language Hearing Association (ASHA).
