As an active employee under the Welfare Plan,
you will be eligible for the Death Benefit, the Accidental Death and
Dismemberment Benefits,
and the Weekly Accident and Sickness Benefits. You, your spouse, and
your dependent children will be eligible for Comprehensive Major Medical,
Optical (Vision), Hearing Aid and Prescription Drug Benefits. Only you
and your spouse will be eligible for Wellness Screening Benefits and
the Infertility coverage under the Comprehensive Major Medical Benefits.

Benefits
The Welfare Plan provides the following Death
and Accidental Death and Dismemberment Benefits in the event of your
death and/or accidental injury. No benefits are payable for death resulting
from suicide or intentionally self-inflicted injury, unless it is a
result of a physical or mental health condition.
